More about Ativan
Ativan product details
Ativan is a brand of lorazepam, a benzodiazepine available in oral and injectable products. Oral tablets are labeled for anxiety disorders or short-term anxiety-symptom relief, including anxiety associated with depressive symptoms. Injectable lorazepam has distinct uses, including status epilepticus and preanesthetic medication in monitored care. The same active ingredient does not make the routes interchangeable. This reference emphasizes oral treatment while identifying the separate clinical context of injections.
Generic name lorazepam Brand names Ativan Drug class Benzodiazepine US federal status Schedule IV Available forms Tablets: 0.5, 1, 2 mg; injectable formulations for clinical use. 02How it works Lorazepam enhances the inhibitory action of GABA at GABA-A receptor complexes. Its effects include anxiety reduction, sedation, impaired memory and anticonvulsant activity. These overlapping effects explain why alertness and coordination can decline even when anxiety feels improved. It does not remove the need to assess depression or the longer-term cause of anxiety. Other substances that reduce CNS activity can magnify lorazepam’s effects on consciousness and breathing, regardless of whether their receptor mechanisms are identical.
Elimination half-life About 12 hours for lorazepam Absorption & metabolism Lorazepam is well absorbed orally and is conjugated to lorazepam glucuronide, which has no demonstrated CNS activity in animal testing. Unlike diazepam, it does not rely on the same oxidative metabolism to long-lived active CNS metabolites. The parent drug’s mean plasma half-life is around 12 hours, with variability by patient and context. Glucuronide is eliminated through the kidneys. This metabolic difference can inform a clinician’s choice but does not make lorazepam free of accumulation, respiratory risk or the need to review liver and renal disease. Half-life describes elimination, not how long benefits last or how long a drug test remains positive. Age, organ function, formulation, and other medicines can change these estimates.
03Medical uses FDA-approved indications Oral Ativan is FDA-approved for management of anxiety disorders or short-term relief of anxiety symptoms, including anxiety associated with depressive symptoms. Injectable Ativan has separate indications including status epilepticus and preanesthetic use. Route-specific labeling should guide interpretation rather than assuming oral tablets treat every injectable indication.
Clinical context & off-label use Catatonia treatment is an off-label clinical use.
04Expected effects The intended oral effect is relief of anxiety. Sedation, slowed reactions and memory impairment may accompany that effect. Injection produces different clinical effects under monitored conditions.
Response differs between individuals. Taking more than prescribed or using someone else’s medication increases harm and is not a reliable way to improve treatment.
05Dosage & administration Prescribing considers the severity and duration of anxiety, age, respiratory status, other sedatives and functional impairment. Older or debilitated patients can be more sensitive, so the clinician may choose a different starting regimen and monitor closely. Oral tablets, oral concentrates and injection products have distinct directions and settings. Injectable use requires trained staff and appropriate monitoring. Continued need should be reassessed rather than assuming the original prescription remains appropriate indefinitely, and a discontinuation plan is needed after sustained use.
Forms & strengths Tablets: 0.5, 1, 2 mg; injectable formulations for clinical use. Prescribing considerations Caution with sleep apnea, COPD, older age and depression; acute narrow-angle glaucoma is a contraindication. This overview is not a dosing schedule. Follow the exact prescription label and product Medication Guide; a clinician or pharmacist should resolve missed-dose questions and formulation changes.
06Side effects & risks Common side effects Sedation, dizziness, weakness, unsteadiness.
Serious reactions & long-term concerns The principal serious concerns are respiratory depression with opioids or other depressants, abuse and addiction, and potentially life-threatening withdrawal. Falls, severe sedation, paradoxical agitation and impaired driving are additional risks. Underlying sleep apnea or COPD can worsen vulnerability to respiratory effects. Acute narrow-angle glaucoma is a labeled contraindication for oral Ativan. Pregnancy and breastfeeding require a separate clinical benefit-risk discussion. Severe breathing changes, unresponsiveness or seizures need immediate help, even when a medicine was taken from a legitimate prescription.
Know when to get urgent help Call 911 for slowed or stopped breathing, inability to wake, seizure, collapse, or severe allergic swelling. If an opioid overdose is suspected, give naloxone if available and follow its instructions while emergency help is on the way. 07Drug interactions Opioids, alcohol, other benzodiazepines, sleeping medicines and sedating antihistamines can increase CNS depression and breathing risk. Valproate can increase lorazepam exposure by reducing clearance, and probenecid can also affect its disposition; these require product-label-guided clinical management. Clozapine combinations warrant particular caution because marked sedation and other serious effects have been reported. A shorter interaction list than that of a CYP-metabolized drug does not establish safety. Review nonprescription sleep aids and all routes of sedative administration with a pharmacist.
Give your pharmacist a complete list of prescriptions, over-the-counter medicines, supplements, and alcohol or other substance use. Do not assume a combination is safe because both medicines were prescribed.
08Dependence, tolerance & withdrawal Continued lorazepam treatment can produce physical dependence. Abrupt stopping or rapid reduction may cause anxiety, insomnia, tremor, agitation, abnormal sensations, delirium or seizures, and some symptoms may persist. Risk generally increases with longer duration or greater exposure, but no simple cutoff guarantees safety. The prescriber should tailor a gradual reduction and monitor the original anxiety condition as well as withdrawal. Lorazepam’s lack of long-lived active metabolites does not protect against severe withdrawal, and changing to another benzodiazepine requires a separate clinical plan.
Physical dependence is an adaptation to regular exposure; it is different from addiction, which involves impaired control and continued use despite harm. A clinician can assess both and plan a gradual reduction when appropriate.
09Pricing & availability Ativan prescription costs vary by strength, formulation, quantity, pharmacy, insurance coverage, and whether a generic is dispensed. A brand name in this reference does not guarantee that the brand is currently marketed or stocked.
Current cash prices have not been independently verified; no price estimate is provided. Ask a licensed pharmacy for a dated quote, compare your insurance copay with legitimate discount offers, and check eligibility for manufacturer assistance. Discount cards are not insurance.
Compare Medicare prescription coverage ↗ 10Legal status in the United States US federal classification: Schedule IV. The medication discussed here has FDA-approved prescription formulations. Approval applies to specific products, routes, indications, and patient groups; it does not endorse every clinical use.
Possession and dispensing must comply with prescription and controlled-substance rules. State requirements can be stricter than federal rules; consult a pharmacist or state authority for your jurisdiction.
11Ativan vs Valium Lorazepam is primarily glucuronidated to a metabolite without demonstrated CNS activity, while diazepam undergoes oxidative metabolism and has long-lived active metabolites. Diazepam can therefore persist and accumulate differently, especially with older age or liver disease. The two medicines have overlapping anxiety uses but route-specific and product-specific differences in seizure and procedural care. Both are Schedule IV benzodiazepines with substantial interaction and withdrawal risks. Selecting one is a clinical decision based on the indication, organ function and treatment duration, not a simple preference for a shorter- or longer-acting drug.
